Welcome to the CIS application page! The following information is designed to assist you with the formal admissions process. The Admissions process officially begins when you submit your student’s Application for Admission to Columbia Independent School (CIS). You may complete the online application form and submit it electronically, taking care to print and send in all relevant supplemental forms; or you may print the application form and mail it in with appropriate supplemental material; or you may request that a hard copy of the full application be mailed to you.
Once the application form has been submitted, along with the signed Transcript Request Form (Supplemental Form 1), the Director of Admissions will contact the applicant’s family to confirm their receipt.
Next, the applicant should provide current teacher(s) with the Teacher Evaluation form(s). Applicants to the Lower School should use Supplemental Form 2, while applicants to the Middle and Upper School should give Supplemental Form 2 to an English teacher and Supplemental Form 3 to a math teacher. The teacher(s) must mail the form(s) directly to CIS. You may wish to provide the teacher(s) with a stamped envelope addressed to Columbia Independent School.
Applicants for grades 6-12 should register to take the entrance examination by calling the Admissions Office at 777-9257.
When all of the aforementioned forms have been received, the Director of Admissions schedules an interview for the applicant and his or her parents/guardians. Applicants for grades 6-12 also participate in a half-day student visit, which may be scheduled for the same day. At the conclusion of the interview (and half-day visit), the applicant’s file is complete.
Finally, the Admissions Committee reviews the file and recommends to the Head of School whether or not the candidate is a good fit for CIS. You will be informed of the committee’s decision by letter or by phone.
